Download Annual Emission Report Forms

These forms and instructions are Adobe Acrobat PDF documents. You will need the free Acrobat Reader software, available from Adobe's web site, to view these documents. Get Acrobat Reader

To download the forms to your computer, click on the form name with the right mouse button and choose "Save Target As..." in Internet Explorer, or "Save Link As..." in Netscape. You will see a "Save As" box pop up with the file name highlighted. If the file name is blank, make sure you save the file with a .pdf extension so that Acrobat will recognize it.

The blank forms have been modified so that they can be filled out and saved electronically to your computer. After downloading, open the form, fill it out and then choose "Save A Copy" and give it an appropriate name. You can then print the pages and submit them with your Annual Emission Report.

Annual Emission Report Rule

Instructions
These are the instructions for all sources that are required to submit an Annual Emission Report.
Address/Source Emissions
This form is used to report company information and source-wide emissions. This form is required to be submitted for all types of annual emission reports.
Combustion Emission Units
This form is used to report data for fuel combustion emission units. It only needs to be completed for sources required to submit the "long" annual emission report form.
Process Emission Units
This form is used to report data for process emission units (reactors, crushers, coaters, etc.). It only needs to be completed for sources required to submit the "long" annual emission report form.
Storage Tanks
This form is used to report data for storage tanks. It only needs to be completed for sources required to submit the "long" annual emission report form.
Other Emission Units
This form is used to report data for emission units that are not combustion units, process units or storage tanks. Typically, emissions units in this category will be fugitive in nature (storage piles, roadways, clean-up solvent, etc.). It only needs to be completed for sources required to submit the "long" annual emission report form.
Emission Unit Emissions
This form is used to report emissions from combustion units, process units, storage tanks or other emission units. For every one of those forms completed, the emissions form must also be completed. It only needs to be completed for sources required to submit the "long" annual emission report form.
Controls
This form is used to report data for control devices and control methods. It only needs to be completed for sources required to submit the "long" annual emission report form.
Stacks
This form is used to report data for stacks and exhaust points. It only needs to be completed for sources required to submit the "long" annual emission report form.
Coating/Solvent Use
This form is used to report data on coating and solvent use. It only needs to be completed for sources required to submit the "long" annual emission report form.
Malfunctions
This form is used to report data on emissions from startups, shutdowns and malfunctions. It only needs to be completed for sources required to submit the "long" annual emission report form.
