Manifest Form Selection Information

The information below is intended to help you determine if you need to use a manifest.  Manifests may be ordered by mail by using the guide listed below, or online via credit card.  Uniform Hazardous Waste Manifest Forms mandated by USEPA were effective September 5, 2006.

Hazardous Waste

is waste or a combination of wastes, which because of quantity, concentration, or physical, chemical, or infectious characteristics may cause or significantly contribute to an increase in mortality or an increase in serious, irreversible, or incapacitating reversible, illness; or pose a substantial present or potential threat to human health or to the environment when improperly treated, stored, transported or disposed of, or otherwise managed, and which has been identified, by characteristics or listing, as hazardous pursuant to Section 3001 of  the Resource Conservation and Recovery Act of 1976 (42 U. S. C. 6901 et seq.) or pursuant to agency guidelines consistent with the requirements of the Act and Board regulations.

You will need a manifest if your company generates hazardous waste.

Non-Hazardous Special Waste

is industrial process waste or pollution control waste that has not been identified, by characteristics or listing, as hazardous pursuant to Section 3001 of the Resource Conservation and Recovery Act of 1976 or Board Regulations.

You will need a manifest if your company generates nonhazardous special waste.

PIMW - (Potentially Infectious Medical Waste)

is generated in connection with the diagnosis, treatment (i.e., provision of medical services), or immunization of human beings or animals; research pertaining to the provision of medical services; or the production or testing of biological materials (section 3.84 of the Act).
