LPC-PA11 Closure Plans and Post-Closure Care Plans
Any facility which is required to have a closure plan or
post-closure care plan as well as any modification or supplemental permit
request which results in a change or update to an existing closure or
post-closure care plan should use this form. This form is to be used for a
closed landfill under 35 IAC 807.
